Sara Vael drew the mercy line in her head before she finished crossing the bay.
It was the first thing they taught you and the last thing you forgave yourself for: in a room with more wounded than hands, you decided who you would try to save and who you would only sit beside. You drew the line, and then you lived on the wrong side of it for the rest of your life.
Eleven men and women lay bleeding across the deck of Aft Surgical Two, and the Meridian was still ringing like a struck bell.
The engagement was nine minutes old and already over — that was the thing about the void, the way it killed you in the time it took to notice it was happening. A Sanctum cutter had come out of a fold-shadow off the port quarter and opened the frigate’s flank like a tin before the gunnery crews finished their tea. Now the ship listed three degrees she would never list again, the gravity in the aft sections ran a half-second behind itself so that everything felt subtly dropped, and the air carried the particular smell Sara had learned to hate more than blood: scorched coolant and the ozone tang of a hull that had been, very briefly, open to nothing at all.
“Vael.” Yusra came skidding up beside her, gloves already on, eyes too wide and too steady at once — the look of a junior medic who had decided to be brave instead of being sick. “Decompression’s flooding us. Sael says the forward bay’s gone.”
“Then we’re the bay.” Sara didn’t slow. She let the room arrange itself in her vision the way it always did, not as people but as colors — red for the ones bleeding out, yellow for the ones who could wait, the terrible quiet gray of the ones already past her. “Talk to me.”
“Crewman Idah, sucking chest wound, port side, I’ve got an occlusive on but he keeps—“
“Pressure’s dropping, he’s tensioning. Needle decompress, second intercostal, then chest seal with a flutter. Go.” She said it without breaking stride, and Yusra peeled off toward the red shape thrashing against the deck, and that was one. One drawn back across the line by hands that weren’t Sara’s, which was the whole point of teaching: you made more hands.
She reached the table.
The man on it had been a gunner an hour ago. Now he was a problem in arterial bleeding and a face she made herself look at, because you owed them that, you owed them your eyes even when you had nothing else. His thigh had been opened to the bone by a spall fragment and he was pumping his life out onto the deck in a rhythm she could have set a metronome to.
“Tourniquet’s not holding,” said the orderly across from her, and he was right, it had slipped on the slick of it, and the femoral was a fire hose, and Sara had perhaps ninety seconds before the color of him changed from red to gray.
She could open him, clamp the vessel, pack it, and pray. With the kit she had and the hands she had, in this listing room, she gave it maybe one chance in three.

The or lived in her always, a second pulse under her pulse. It would have been faster to say she was tempted; the truth was worse and quieter than temptation. The or was simply there, the way a door is there in a wall, and every single time she had to choose to keep walking past it.
She glanced up. Two beds down, half-shadowed by the failing overhead, a man in an immaculate gray coat stood with his hands folded and his head tilted, watching the room the way an auditor watches a ledger. Curate-Surgeon Aldous Pell had come aboard at the last station rotation as a political-medical observer, which was three words for a man whose job no one could quite name and whose attention landed on the wounded like a cold draft. He was not looking at the gunner. He was looking, with mild and patient interest, at a casualty near the far bulkhead whose skin had the faint bruised sheen Sara had learned to recognize and never name aloud — a Resonant, one of theirs, burning even now, even dying.
Pell had a slim case open at his hip and was, she saw, drawing something from the unconscious man’s arm into a vial. Not a treatment. Treatments went in.
She looked back down at her gunner, because that was where she was allowed to look.
“Reload the windlass,” she told the orderly. “On three, you release, I pack. One. Two—“
On three she put her bare fingers into the wound.
That was permitted; that was field medicine; everyone’s eyes were on the spurt and the gauze and the practiced violence of her packing it down. No one watched the place where her skin met his, and no one — no one ever — could see the thing that happened there, because it happened a layer beneath seeing.
She reached.
It was nothing like the way the Sanctum sang about it in their broadcasts, all light and choirs. It was closer to threading a needle in the dark by feel: she found the torn vessel from the inside, the way you’d find a frayed wire with your fingertips, and she told the smooth muscle of its wall to clench, told the ragged endothelium to remember it had once been whole and to reach for itself, told the clotting cascade — already drowning, already failing — to hold the line right here. Not magic. Biology, hurried. Biology, leaned on. Cells doing what cells did, only faster and on purpose, because something outside them had asked.
And the cost arrived, the way it always arrived, instant and intimate and exactly fair.
The migraine came first — a white spike driven through her left eye that made the room tilt to match the ship. Then the warm-copper trickle at her nostril she’d been expecting, that she caught with a practiced two fingers pressed to the bridge of her nose, the gesture that read as nothing but a tired woman steadying herself. And under her sternum, worst because it was hers and no one else’s, her heart skipped — once, twice — a little stutter-step, a missed beat that the Corps would have called an arrhythmia and that she called the toll. The Chorus didn’t give. It only routed. To mend a body you ran the work through your own, and the body that did the work always paid. She had been paying for fourteen years. Her tally ran somewhere she didn’t let herself look.
The gunner’s bleeding stopped.
“Clamp’s holding,” the orderly breathed, not understanding that there was no clamp, that her packed gauze was a story they were both telling the wound while the real work knit beneath it. “God. Doc. I thought he was gone.”
“He’s not gone.” Sara took her fingers out, gloved up over the blood, let the gauze take the credit. “Tourniquet at fifty, get him to the line for surgical revision, he keeps the leg if we’re lucky.” She was already pressing her sleeve to her nose under the guise of wiping her face. “Recycled air in here’s a crime. Somebody tell Environmental.”
She did not let herself look at the far bulkhead, where Pell capped his little vial and wrote something on a slate, and where the Resonant man whose blood he’d taken died quietly between her looking and not looking, unattended, while a healer with secret hands stood eleven meters away and could not cross to him without crossing her own line too.
You can’t save everyone, Doc. Yusra had said it once, kindly, when Sara had stayed up two nights running with a fever patient who’d died anyway. You’ll burn yourself down trying. The girl had no idea how literally she’d aimed.
She drew the line again. She lived on the wrong side of it. She moved to the next red shape on the deck.

It went like that for three hours.
The list of the dead grew and the list of the saved grew faster, which on a medical frigate was the only victory there was. Yusra held her chest wounds. The aft bay stabilized as Environmental got a patch over the breach and the gravity stopped lagging, settling into something the inner ear could forgive. Sara mended twice more, both times where no one watched, both times paying — a deeper nosebleed the second time, a few seconds where her own vision grayed at the edges and she had to grip the table and call it fatigue, which it also was. She had learned a long time ago that the safest place to hide a forbidden gift was inside a licensed pair of hands. A surgeon could touch anyone. A surgeon was supposed to have her fingers in the wound.
Somewhere in the third hour she allowed herself ten seconds. She stepped into the clean-supply nook between the autoclave and the bulkhead, out of the cameras’ eye, put her forehead against the cold steel of the shelving, and let her hands shake — really shake, the way they were never allowed to shake in the open — and counted. One. Two. The migraine sat behind her eye like a coal. Five. Six. Her heart did its little stutter and caught. Nine. Ten. Then she straightened and wiped her face, and the shaking went back into the drawer with everything else, and she walked out into the red and the gray and was Surgeon-Lieutenant Vael again, whom no one aboard had ever seen weep.
It was the Accord’s own cruelty that made the hiding work. They had decided, forty years ago and with the best intentions a frightened government ever has, that people like Sara were a condition — Resonance-Positive, RP, a pathology to be registered and dampered and fitted with a shunt that capped the gift and tracked the body and, if it came to it, switched the body off. Humane on paper. She had read the white papers. She had also seen the containment wards, the rows of beautiful diminished people sitting in clean light with the lights gone out of them, and she had made her choice between safe and free the way you make a choice you can never take back: quietly, alone, and forever.
So she was Surgeon-Lieutenant Vael, unregistered, no shunt, hiding in plain sight inside the very machine built to find her. The monitors were the hard part, and she’d beaten them the slow way, years ago: her own intake file doctored to read baseline, her cardiac profile logged during a brutal stim-shift so that her resting rhythm always ran a little ragged, a little overworked — like a woman who pulled too many doubles, never like a woman spending her own heart to buy back someone else’s. The arrhythmia hid inside the overwork. The nosebleeds hid inside the recycled air. You didn’t hide a thing like hers behind one good lie; you hid it behind a hundred small dull ones and kept them all, every day, for fourteen years. It was the most dangerous thing in the room, and it spent its life saving the room, and no one knew.
She was elbow-deep in the last revision, closing the gunner’s thigh properly now with suture and not with secrets, when the bay’s outer hatch cycled and Captain Doreh Sael came through it with the particular stride of a woman bringing news no one wanted.
“Vael.” Sael was gray-haired, square-built, a hardliner by reputation and, underneath it, just tired — one of the decent ones, which Sara had decided long ago was worse than the cruel ones, because you couldn’t even hate them clean. “We took prisoners.”
“We took a Sanctum cutter’s prisoners?”
“We took the cutter’s cargo.“ Sael’s jaw worked. “It was a transfer run. Free Clade combatants, captured off Maro Drift two days ago, being moved to a Sanctum sanctuary.” The word came out of her like something spat. They both knew what sanctuary meant for a Resonant who went into one. “We cracked the cutter before it could fold out. There are nine of them in the hold. Six are walking. Two are not. And one—“ She stopped.
“One,” Sara prompted, tying off, not looking up.
“One is their field-commander, and he’s dying, and I am told he is a Kinetic.” Sael said Kinetic the way you said the name of a weapon. “Which means he’s a combatant, a flight risk, and a Resonant burning out in my hold, and the regs say I isolate him and let nature take its course.” Her voice did not change. Only her eyes did. “I’d like him to live long enough to answer questions. Pell would like him to live long enough for other reasons, and I trust those reasons about as far as I can throw the cutter we just killed.” A beat. “You’re the only surgeon I’ve got who doesn’t flinch. Take him.”
Every rule Sara had built her life out of told her to say yes, Captain and nothing else.
A Kinetic. Burning. The most-watched, most-wanted kind of patient in the void, the kind Pell would hover over with his little vials, the kind whose every monitor command would be logged and reviewed — and they wanted to put him under the hands of the one surgeon aboard who could not afford to be looked at closely, ever, for any reason.
“Where,” Sara said, because yes, Captain was already decided; it had been decided fourteen years ago, in a watched room, when she chose her secret over a dying man and swore — uselessly, to no one — that she would spend the rest of her life refusing to make that choice again. Take him. As if she would have done anything else. As if she had ever once, since that day, looked at the wrong side of the mercy line and not ached to cross it.
“Brought him to the isolation berth,” Sael said. “He’s restrained. He’s also—“ The captain almost smiled, and it was not a kind smile, it was the smile of someone passing along a problem. “He’s awake. And he has opinions about us.”
They went together down the listing corridor, past the patched breach still weeping frost at its edges, to the small sealed room where the Meridian kept the patients it was afraid of.
He was strapped to the berth with restraints meant for the violent, and he did not look violent. He looked like a man who had been beautiful once and had spent it. Broad shoulders gone lean, a braid she didn’t recognize the meaning of yet woven tight against his skull, knuckles scarred from work and not from fighting. His eyes were the worst of it — the whites gone scarlet at the edges with the burst-vessel bloom she knew too well, the sign of a Resonant who had channeled past every safe limit and kept going. A right hand that trembled where it lay open against the strap, fine and constant, the tremor of someone rationing his own heartbeats and losing the count.
He was, she understood with a lurch that had nothing to do with the ship, exactly what she was. He was simply honest about it, and dying for the honesty, where she had hidden and lived.
His scarlet-edged gaze tracked her across the room — found the blood drying at her cuffs, the surgeon’s economy of her, the two-fingered ghost of a gesture she didn’t know she still wore at the bridge of her nose — and his cracked mouth curved into something that was almost a laugh and entirely without warmth.
“They sent the mender,” Tovan Reach said, in the dry burr of the Clades, looking at her as though he could see straight through to the thing she’d hidden for fourteen years. “Come to keep me alive for the people who’ll cage me. That’s the Accord all over.” His ruined eyes held hers, steady, contemptuous, and underneath the contempt a flicker she had no business reading the way she read it — because there was no way for him to know what she was, no way in the universe — and she read it as recognition anyway, and could not make herself stop. “Tell me, Surgeon. Does it ever stop being a mercy and start being the trap, or do you people not let yourselves notice the difference?”
Sara reached for the monitor. Her hand, she was distantly furious to discover, was not quite steady either.
“Lie still,” she said. “You’re bleeding into your own skull, Commander, and I’m the only person on this ship who can stop it.”
“Then I’m already dead,” said Tovan Reach, and smiled, and meant it.
And there it was — the one wound she had never, in fourteen years, been able to walk past: a person who had decided to die.
She knew what the rules wanted. Stabilize the skull-bleed, keep him breathing for Sael’s questions, feel nothing. She knew, too, the thing the rules didn’t — that the ones who’d quit slipped through every set of hands, hers included, secret or not; that a body would fight for you, but a man who had laid his own weapons down would not.
Instead she heard herself already beginning the oldest argument she knew, the one she always lost, for why this one would be different. For why she was going to make a man who wanted to die want to stay.
She did not yet have the sense to be afraid of how much she meant it.
